• If you have tested positive, and have NOT had a consecutive NEGATIVE test, we at Dr. Mizgala’s office, will be unable to treat you due to the safety of our staff and patients.
• If you are experiencing ANY COVID 19 symptoms currently or in the past 14 days, (for example: Loss of taste or smell, cough, fever, chills, nausea, vomiting, diarrhea or Shortness of Breath), we are unable to treat you until ALL symptoms are alleviated AND a noted Negative test.
• If you have been in close contact with an individual who is infected with the COVID 19 virus, including individuals who are experiencing the symptoms listed above, within the past 14 days, we are unable to treat you until the allotted time has passed.
• If you have traveled to any areas in which there are Level 2, 3, or 4 travel advisories which are place by the U.S. State Department, we are unable to treat you.
• All patients will be screened upon entering office.
